Based on the findings of an onsite unannounced Medicare Recertification and State Licensure survey conducted 8/30/2022 through 9/02/2022, Lakeland Area Hospice, LP was found to be in compliance with the requirements of 42 CFR, Part 418.113, Subpart D, Conditions of Participation: Hospice Care - Emergency Preparedness.
